Provider Payments

All attendance forms are due to the Provider Payment Department no later than the 3rd business day of each month for the prior month. Attendance forms received after the 3rd business day are late. Late attendance may affect current payment.

You may submit your attendance forms to the Provider Payment Department by mail, fax, or self delivery.
