Provider Reimbursement

All attendance forms are due to the Provider Reimbursement Department no later than the 3rd calendar day of each month for the prior month's attendance. Attendance forms received after the 3rd calendar day are late. Late attendance may affect current payment.

You may submit your attendance forms to the Provider Reimbursement Department by mail, fax, or self delivery.
